Our staff’s goal is to provide youwith the best care for all your orthopedic needs. If you have any pertinent records, such as x-rays, prior procedures, or surgeries, etc, call your corresponding physician’s office to request a copy of these records. This additional information is very important and will assist our physicians in determining the treatment plan most appropriate for you. Prior to your appointment, please make sure of the following: • Referrals or authorizations have been obtained. • Necessary insurance forms and insurance cards must be brought with you. • Please keep in mind that your copayment is due at the time of your appointment. • Medical records and/or radiology films from other physician offices have been requested and accompany you to your appointment • You have a list of questions that you may want to ask your doctor. • You have a list of medications you currently are taking. • If it becomes necessary to cancel this appointment, please call the office to let us know as soon as possible. • If you are being treated for a worker’s comp injury, please inform the receptionist as you will need to get prior approval before your visit date. Please check with our Worker’s Compensation department for any questions. Appointments that you do NOT have prior approval for will be rescheduled.
